少しブログ更新の間が空いてしまいましたが、今日は腰椎椎間板ヘルニアについて書いてみたいと思います。

「お尻から足にかけて激しい激痛やしびれがある」

「腰が痛くて屈めない、歩くのがつらい」

このような症状で当院にご相談いただく患者さんも数多くいらっしゃいます。

「腰椎椎間板ヘルニア(ようついついかんばんへるにあ)」と診断されると、「すぐに手術が必要なのだろうか…」と不安になる方もいらっしゃるかと思います。

最近の研究・論文では、腰椎椎間板ヘルニアの多くは手術をせずに治る(または自然に縮小する)ことが明らかになってきています。さらに、2018年に登場した切らない治療薬「ヘルニコア」をはじめ、治療の選択肢は大きく広がっています。

今回は最新のエビデンスに基づき、腰椎椎間板ヘルニアの原因や症状、そして「保存療法」「注射療法(ヘルニコア)」「手術療法」の3つの治療法について書いてみたいと思います。

  1. 腰椎椎間板ヘルニアとは?原因と主な症状

腰椎椎間板ヘルニアのメカニズム

背骨(脊柱)の骨と骨の間には、クッションの役割を果たす「椎間板(ついかんばん)」が存在します。

椎間板は、中央にあるゼリー状の「髄核(ずいかく)」と、それを囲む硬いコラーゲン線維の「線維輪(せんいりん)」からできています。

加齢や日常的な腰への負担(中腰での作業、長時間のデスクワーク、重いものを持つ動作など)によって線維輪に亀裂が入ると、中の髄核が押し出されて飛び出してしまいます

この飛び出した髄核がすぐ後ろを通る神経を圧迫し、強い痛みやしびれを引き起こすのが「腰椎椎間板ヘルニア」です。

 

主な症状と「坐骨神経痛」

  • 激しい腰痛・お尻の痛み
  • 太ももから足先にかけての「痛い・痺れる」(坐骨神経痛)
  • 前屈(お辞儀)をすると痛みが強まる
  • 足に力が入りにくくなる(筋力低下)

 

すぐに病院受診が必要な「危険なサイン」

次のような症状が出ている場合は、神経の根本(馬尾神経)が強く圧迫されている可能性(馬尾症候群)があり、緊急手術が必要となるケースがあります。

  • 排尿・排便のコントロールができない(尿が出にくい、漏れてしまう)
  • お尻の周りや股間の感覚が鈍い(サドル麻痺)
  • 足首や足の指が持ち上がらず、歩行時に引っかかる(下肢麻痺)

 

  1. 治療の第1選択:切らないで治す「保存療法」

最新の国際的な診療ガイドラインや大規模データ解析論文においても、危険な麻痺症状がない限り、「まずは保存療法を行うこと」が強い推奨(第一選択)とされています。

飛び出したヘルニア(髄核)の多くは、体内の免疫細胞(マクロファージなど)によって「異物」とみなされ、発症から2〜3ヶ月ほどで自然に吸収されて小さくなる(自然退縮する)性質があるためです。

主な保存療法の種類

  1. 薬物療法
    • NSAIDS(消炎鎮痛剤): 炎症と痛みを抑える基本の薬です。
    • 神経障害性疼痛治療薬: 神経の過剰な興奮を抑え、ビリビリとしたしびれや痛みを和らげます。
  2. 神経ブロック注射
    • 痛みの原因となっている神経の近くに局所麻酔薬やステロイドを注射し、強い痛みの伝達と炎症を直接ブロックします。
  3. 理学療法・リハビリテーション
    • 急性期の激しい痛みが落ち着いた段階で始めます。かつては「長期間の絶対安静」が推奨されていましたが、近年の研究では「過度な安静はかえって回復を遅らせる」ことが証明されています。
    • 痛みのない範囲で体を動かし、体幹の筋力を整えることが早期回復につながります。
  1. 保存療法と手術の中間:椎間板内酵素注入療法「ヘルニコア」

「薬や注射を数ヶ月続けても痛みが引かない」

「でも、手術でメスを入れるのには抵抗がある…」

このような方のために開発され、日本国内で2018年に承認された画期的な治療が「ヘルニコア(一般名:コンドリアーゼ)」を用いた椎間板内酵素注入療法です。

入院しての治療が必要なため当院で行うことはできませんが、適宜専門病院を紹介させていただきます。

ヘルニコアの仕組みと特徴

ヘルニコアに含まれる酵素(コンドリアーゼ)をヘルニアがある椎間板内に直接注射します。

この酵素が髄核の中にある水分を保持する成分(プロテオグリカン)を特異的に分解します。

髄核の水分保持能が低下することで椎間板の内圧が下がり、飛び出していたヘルニアが縮小して神経への圧迫が軽減されます。

ヘルニコア治療のメリットと注意点

【メリット】

  • 体に優しい(低侵襲): レントゲン装置で位置を確認しながら注射をするため、メスを使いません。
  • 短期間で復帰できる: 日帰りまたは1泊2日程度の短い入院で施工可能です。
  • 全身麻酔が不要: 局所麻酔で行えるため、身体への負担が少ないです。

【注意点・適応条件】

  • 生涯に1度しか受けられない: アナフィラキシーなどのアレルギー反応を防ぐため、一生に1回のみ投与可能です。
  • すべてのヘルニアに効くわけではない: 「後縦靱帯下脱出型(こうじゅうじんたいかでっしゅつがた)」と呼ばれる、椎間板の膜のなかにとどまっているタイプのヘルニアが対象です。完全に膜を破って外に飛び散ってしまったヘルニアには効果が期待できません。
  • 投与後の一時的な腰痛: 髄核が収縮する影響で、投与後数週間〜1ヶ月ほど腰に重苦しさや痛みが一時的に出ることがあります。
  1. 確実性を追求する「手術療法」

保存療法やヘルニコアを行っても十分な効果が得られない場合や、足の麻痺・排尿障害などの重大な症状がある場合は「手術療法」が検討されます。

近年の手術のトレンドは、「最小侵襲」で、骨や筋肉へのダメージを最小限に抑える技術が飛躍的に進化しています。

主な手術法

  1. 全内視鏡下椎間板切除術(FELD / PELD法など)
    • 直径数ミリ〜1センチ弱の極小の内視鏡カメラを背中から挿入し、モニターで見ながらヘルニアだけをピンポイントで摘出します。傷口が非常に小さく、術後数日〜1週間程度で社会復帰が可能です。
  2. 顕微鏡下椎間板切除術(MD法) / 顕微鏡下切除
    • 数センチ切開し、拡大顕微鏡で神経とヘルニアを立体的に確認しながら安全に摘出します。確実性が高く、昔から実績のある手術方法です。

Hello, everyone! I’m Dr. Kurimoto, the Director of One Clinic Kojimachi, an orthopedic clinic located on the 2nd and 3rd floors along Shinjuku-dori, right between Kojimachi Station and Hanzomon Station in Chiyoda-ku, Tokyo!

It’s been a little while since my last post, but today I’d like to write about lumbar disc herniation.

  • “I have severe pain and numbness radiating from my buttocks down to my legs.”
  • “My lower back hurts so much that I can’t bend over or walk comfortably.”

We see many patients at our clinic who come to consult us with these exact symptoms. When people receive a diagnosis of lumbar disc herniation, many worry, “Does this mean I need surgery right away?”

Recent studies and papers have revealed that many cases of lumbar disc herniation resolve (or shrink naturally) without surgery. Furthermore, since the approval of “Hernicore”—a non-surgical injectable treatment introduced in 2018—treatment options have expanded significantly.

Based on the latest evidence, I would like to explain the causes and symptoms of lumbar disc herniation, as well as three main treatment options: Conservative Therapy, Injection Therapy (Hernicore), and Surgical Therapy.

  1. What is Lumbar Disc Herniation? Causes and Main Symptoms

The Mechanism of Lumbar Disc Herniation

Between the bones (vertebrae) of the spine, there are “intervertebral discs” that act as shock absorbers. An intervertebral disc consists of a jelly-like center called the nucleus pulposus surrounded by a tough outer ring of collagen fibers called the anulus fibrosus.

When tears develop in the anulus fibrosus due to aging or daily stress on the lower back (such as working in a stooped position, prolonged desk work, or lifting heavy objects), the nucleus pulposus inside gets pushed outward. Lumbar disc herniation occurs when this protruding nucleus pulposus presses against nearby nerves, causing severe pain and numbness.

Key Symptoms and “Sciatica”

  • Severe lower back and buttock pain
  • Pain or numbness extending from the thigh down to the toes (sciatica)
  • Increased pain when bending forward
  • Difficulty applying strength to the leg (muscle weakness)

“Red Flags” Requiring Immediate Medical Attention

If you experience any of the following symptoms, the nerve roots at the end of the spinal cord (cauda equina) may be severely compressed (Cauda Equina Syndrome), which may require emergency surgery:

  • Loss of bowel or bladder control (difficulty urinating or incontinence)
  • Numbness around the buttocks or groin area (saddle anesthesia)
  • Inability to lift the ankle or toes, causing the foot to catch while walking (lower limb paralysis)
  1. The First Choice: Non-Surgical “Conservative Therapy”

Even in the latest international clinical guidelines and large-scale data analysis papers, starting with conservative therapy is strongly recommended as the first-line treatment, provided there are no dangerous symptoms of paralysis.

This is because the body’s immune cells (such as macrophages) often recognize the protruding herniated nucleus pulposus as a “foreign object,” causing it to naturally absorb and shrink (spontaneous regression) within about 2 to 3 months after onset.

Main Types of Conservative Therapy

  • Pharmacotherapy (Medication)
    • NSAIDs (Anti-inflammatory analgesics): The foundational medication used to reduce inflammation and pain.
    • Neuropathic Pain Medications: These suppress excessive nerve excitation, alleviating tingling pain and numbness.
  • Nerve Block Injections
    • Local anesthetics and steroids are injected near the affected nerve to directly block strong pain signals and reduce inflammation.
  • Physical Therapy & Rehabilitation
    • Started once severe pain in the acute phase subsides. While “prolonged bed rest” was recommended in the past, recent studies have proven that excessive rest can actually delay recovery. Moving your body within a painless range and strengthening your core muscles leads to a faster recovery.
  1. The Middle Ground Between Conservative Therapy and Surgery: Intradiscal Enzymatic Injection Therapy “Hernicore”

“My pain hasn’t faded even after months of medications and injections…”

Developed for patients facing this dilemma, Hernicore (generic name: condoliase) is a groundbreaking intradiscal enzymatic injection therapy approved in Japan in 2018.

How Hernicore Works and Its Features

The enzyme contained in Hernicore (condoliase) is injected directly into the herniated intervertebral disc. This enzyme specifically decomposes glycosaminoglycans (proteoglycans), which are the water-retaining components inside the nucleus pulposus.

By reducing the water-retention capacity of the nucleus pulposus, the internal pressure of the disc decreases. As a result, the protruding hernia shrinks, relieving pressure on the nerves.

Benefits and Points to Note regarding Hernicore

【Benefits】

  • Minimally Invasive: It uses no scalpel; the injection is guided accurately using X-ray imaging.
  • Quick Return to Daily Life: It can be performed as an outpatient procedure or during a short overnight stay (1 night, 2 days).
  • No General Anesthesia Needed: Performed under local anesthesia, putting minimal physical strain on the body.

【Points to Note / Eligibility】

  • Can Only Be Administered Once in a Lifetime: To prevent allergic reactions such as anaphylaxis, it can only be administered once per patient’s lifetime.
  • Not Effective for All Types of Herniation: It is indicated for subligamentous herniations (herniations contained beneath the posterior longitudinal ligament). It is not effective if the hernia has completely ruptured through the membrane and migrated.
  • Temporary Post-Injection Back Pain: Due to the shrinkage of the nucleus pulposus, temporary heavy discomfort or pain in the lower back may occur for a few weeks to a month following administration.
  1. Pursuing Certainty: “Surgical Therapy”

Surgical therapy is considered when conservative therapy or Hernicore fails to yield sufficient results, or when severe symptoms such as leg paralysis or urinary dysfunction are present.

Recent surgical trends heavily emphasize minimally invasive techniques, with technology evolving dramatically to minimize damage to bone and muscle tissue.

Main Surgical Methods

  • Full-Endoscopic Lumbar Discectomy (FELD / PELD, etc.)
    • A tiny endoscopic camera (ranging from a few millimeters to under 1 cm in diameter) is inserted through the back to precisely excise only the herniated tissue while viewing a monitor. Because the incision is extremely small, patients can return to normal social activities within a few days to a week after surgery.
  • Microscopic Discectomy (MD) / Microscopic Resection
    • An incision of a few centimeters is made, and the surgeon safely removes the hernia while viewing the nerves in 3D under an magnifying microscope. It is a highly reliable standard surgical procedure with a long track record.
