胸郭・胸椎の柔軟性と自宅でできるストレッチ 〜〜Why Does Neck and Shoulder Stiffness Persist? Rib Cage and Thoracic Spine Mobility and Stretches You Can Do at Home〜〜

↓↓English Below↓↓

Hello, everyone!

I am Dr. Kurimoto, director of One Clinic Kojimachi, an orthopedic clinic on the second and third floors of a building along Shinjuku-dori, between Kojimachi Station and Hanzomon Station in Chiyoda-ku, Tokyo.

We are gradually seeing more chilly days. How have you been?

“My neck and shoulder stiffness has become chronic.” “Even after a massage, it comes right back.” “After prolonged desk work, my neck and shoulders feel heavy.”

Do any of these concerns sound familiar?

Neck and shoulder stiffness may not improve sufficiently simply by relaxing the muscles in those areas. Many factors can contribute, including staying in the same position for long periods, reduced muscular endurance, restricted movement of the rib cage and thoracic spine, and impaired shoulder blade function.

To improve neck and shoulder stiffness, it is important not only to relieve pain but also to assess why strain is concentrated in these areas and improve physical function through appropriate exercise.

In this article, we explain the mechanisms underlying neck and shoulder stiffness, why rib cage and thoracic spine mobility matters, and specific stretches and exercises you can do at home.

1. Why Does Neck and Shoulder Stiffness Occur?

Neck and shoulder stiffness refers to discomfort such as heaviness, tightness, or stiffness extending from the back of the neck to the shoulders and the areas around the shoulder blades.

There is no single cause. In addition to problems involving muscles and fascia, factors such as working posture, lack of exercise, sleep, and stress interact in complex ways.

The Cycle of Muscle Strain and Pain

Muscles around the neck and shoulders, including the trapezius, levator scapulae, and splenius capitis, support the head and shoulder blades. These muscles work continuously during everyday activities to hold the head in position and support the arms.

Prolonged computer or smartphone use can make it more likely that you remain in a forward-head or rounded-back posture. These postures do not necessarily cause neck and shoulder stiffness, but holding the same position for a long time can concentrate strain on certain muscles.

When pain occurs, you may unconsciously tense your muscles or move less often. This can reduce muscular endurance and movement function, sometimes creating a cycle in which strain becomes even more concentrated.

With chronic neck and shoulder stiffness, changes in the nervous system’s pain-regulation mechanisms may also be involved, in addition to muscle tension. It is therefore important to assess movement and lifestyle habits rather than simply massaging the painful area.

2. Why Rib Cage and Thoracic Spine Mobility Matters

The rib cage consists of structures including the thoracic spine, ribs, and sternum, and is involved in breathing and trunk movement. The thoracic spine is the portion of the spine in the chest region and contributes to movements such as extension and rotation.

The shoulder blades, meanwhile, glide over the rib cage and work together with the shoulder joints when you raise your arms.

If thoracic extension or rotation is restricted, other parts of the body may compensate when you raise your arms or twist your trunk. Because this can also affect shoulder blade movement and shoulder joint motion, rehabilitation for neck and shoulder stiffness includes assessing the thoracic spine and rib cage, not just the neck and shoulders.

However, a stiff thoracic spine is not necessarily the cause of neck and shoulder stiffness. In addition to improving mobility, it is important to build strength and muscular endurance around the shoulder blades.

3. Stretches and Exercises You Can Do at Home

Below, we introduce exercises to improve thoracic spine mobility, stretches for the neck and shoulder muscles, and exercises to build muscular endurance around the shoulder blades.

If you have significant pain, do not push yourself. Start within a comfortable range of movement.

① Seated Thoracic Extension

Purpose: To straighten a rounded upper back and encourage thoracic extension.

How to do it

1.     Sit toward the front of a chair with a backrest.

2.     Clasp your hands in front of your chest or rest them lightly behind your head.

3.     Keep your pelvis stable and slowly lift your chest.

4.     If the backrest contacts your back just below the shoulder blades, use that point as a support to gently extend your thoracic spine.

5.     Return to the starting position without bouncing.

Repetitions and duration

·       5–10 repetitions

·       Approximately 2–3 seconds per repetition

·       1–2 sets per day

Precautions

Focus on extending through the chest region rather than excessively arching your lower back. Avoid tilting your neck too far backward.

This exercise is suitable for people with limited thoracic extension or those whose upper back tends to round during prolonged desk work.

② Side-Lying Thoracic Rotation Stretch (Open Book)

Purpose: To improve thoracic rotation mobility and make trunk rotation smoother.

How to do it

1.     Lie on your side with both knees slightly bent.

2.     Extend both arms in front of your body with your palms together.

3.     Slowly open your upper arm while rotating your chest toward the opposite side.

4.     If your neck is pain-free, follow the moving hand with your eyes.

5.     Hold for 3–5 seconds in a comfortable position, then slowly return to the starting position.

6.     Repeat on the other side.

Repetitions and duration

·       5–8 repetitions on each side

·       Hold each repetition for 3–5 seconds

·       1–2 sets per day

Precautions

Avoid letting your knees move significantly or twisting only through your lower back. If you feel pain in your shoulder or back, reduce how far you open your arm.

This exercise encourages thoracic rotation and helps you become more aware of movement throughout the rib cage.

③ Shoulder Blade Movement Exercise (Scapular Retraction)

Purpose: To activate the muscles around the shoulder blades and improve their ability to stabilize the shoulder blades.

How to do it

1.     Sit in a chair or stand upright.

2.     Relax your shoulders and let your arms hang naturally at your sides.

3.     Without shrugging, move both shoulder blades toward your spine.

4.     Hold for 2–3 seconds, then slowly relax.

Repetitions and duration

·       10–15 repetitions

·       Hold each repetition for 2–3 seconds

·       1–2 sets per day

Precautions

Do not squeeze your shoulder blades together too forcefully or arch your lower back to lift your chest. Continue breathing naturally throughout the exercise.

Once you are comfortable with this exercise, you may progress to rowing exercises using a resistance band. Avoid shrugging as you pull your elbows backward, and start with light resistance.

④ Gentle Chin Retraction (Chin Tuck)

Purpose: To adjust head position and improve neck muscle function.

How to do it

1.     Sit in a chair and look straight ahead.

2.     Gently draw your chin straight backward, keeping it level rather than looking down.

3.     Keep the back of your neck long, as though the back of your head were reaching upward.

4.     Hold for 3–5 seconds, then relax.

Repetitions and duration

·       5–10 repetitions

·       Hold each repetition for 3–5 seconds

·       1–2 sets per day

Precautions

Imagine making a slight double chin, without using excessive force. Avoid pulling your chin back so forcefully that you round your neck or look downward.

Stop if neck pain or numbness in your arm increases. This exercise is not necessary for every case of neck and shoulder stiffness; it should be selected after assessing head position and neck muscle function.

⑤ Wall Push-Up Plus

Purpose: To activate muscles such as the serratus anterior and improve shoulder blade stability.

How to do it

1.     Stand facing a wall and place your hands on it about shoulder-width apart.

2.     Bend your elbows slightly to move toward the wall, then slowly push yourself back.

3.     Once your elbows are straight, push a little farther into the wall to move your shoulder blades forward.

4.     Without shrugging, slowly return to the starting position.

Repetitions and duration

·       8–12 repetitions

·       Start with 1 set per day

·       If comfortable, increase to 2 sets per day

Precautions

Do not push through shoulder or wrist pain. Keep your body in a straight line without arching your lower back.

Strengthening the muscles around the shoulder blades is important for supporting shoulder blade movement when you move your arms. However, resistance should be adjusted according to your shoulder condition and exercise experience.

4. How Should You Combine These Exercises at Home?

You do not need to do all the exercises at once. Begin with thoracic extension and rotation exercises and strengthening exercises for the muscles around the shoulder blades, and adjust your routine so that you can continue comfortably.

Even beginners can spend anywhere from a few minutes to about 10 minutes on these exercises. Rather than doing everything from the outset, you can choose two or three exercises suited to your needs and gradually add more while monitoring your symptoms.

You may feel a gentle stretch or muscle fatigue during exercise. However, stop if you experience sharp pain, numbness, or a clear worsening of symptoms.

The effects of exercise also vary from person to person. Significant changes may not occur within just a few days, so it is important to continue while monitoring changes in symptoms and physical function.

5. Adjustments During Desk Work Are Important Too

Even if you stretch and do strengthening exercises, staying in the same position for long periods may concentrate strain on your neck and shoulders.

Keep the following points in mind during desk work:
