急に暑くなりましたが皆様いかがお過ごしでしょうか?今回は暑い時期(ビールが旨い時期とも言えます)によくみられる痛風発作について書いてみたいと思います。
痛風(高尿酸血症)は、かつて「贅沢病」と呼ばれていましたが、現代の日本においては誰もが突然かかる可能性のある、とても身近な病気です。
当院があるビジネス街でも、「朝起きたら足の親指が激痛で歩けない!」と急遽駆け込んでこられる働き盛りの患者さんもいらっしゃいます
今回は、痛風になりやすい人の特徴や最新の遺伝子の話、そして当院での具体的な治療法まで、分かりやすく解説します。
- 痛風になりやすい年齢は?最近のトレンド
現在、日本国内で痛風の治療を受けている人は100万人を超えています。さらに、その予備軍である「高尿酸血症(尿酸値が基準より高い状態)」の人は、成人男性の4〜5人に1人(約1000万人)もいると推定されています。
発症のピークは「30代〜50代の男性」
痛風発作が最も多く起こるのは、まさに働き盛りの30代〜50代の男性です。
- 若年齢化が進んでいる: 以前は50代以降の病気というイメージがありましたが、最近では20代〜30代の若い世代での発症が目立っています。
- 若い頃からの食生活の欧米化(肉の食べすぎなど)や、肥満傾向の人が増えたことが原因と考えられています。
- 高齢の方も油断禁物: 一方で、高齢化に伴い、他の病気の薬(利尿薬など)の影響や、加齢による腎機能の低下が原因で、シニア世代になってから初めて痛風になる方も増えています。
- なぜ圧倒的に「男性」に多いのか?
痛風患者さんの男女比は、なんと95:5以上。圧倒的に男性が多い病気です。これには女性ホルモンが大きく関係しています。
女性ホルモンが守ってくれている
女性ホルモン(エストロゲン)には、尿酸を尿と一緒に体の外へ排泄するのを助ける働きがあります。そのため、閉経前の女性は尿酸値が低く保たれ、痛風になることは滅多にありません。
しかし、閉経を迎えると女性ホルモンが急激に減るため、女性も尿酸値が上がりやすくなります。
実際、女性の痛風患者さんの多くは60代以降です。
ただし最近では、若い女性でもお酒の飲みすぎや、甘いジュース・スイーツの摂りすぎ、過度なダイエットによる水分不足などが原因で尿酸値が高くなるケースもあるため注意が必要です。
- 「食べすぎ」だけじゃない!遺伝子と民族のヒミツ
痛風は「贅沢なものを食べすぎ・飲みすぎた人がなる自業自得の病気」と思われがちですが、実はそれは誤解です。
近年の研究で、体質(遺伝)が強く関係していることが分かってきました。
民族による違い
尿酸値の下がりやすさには、生まれ持った民族的な特徴があります。
- オセアニアの地域: 世界で一番痛風が多いと言われているのが、ニュージーランドのマオリ族などの方々です。
- 彼らは遺伝的に尿酸を体内に溜め込みやすい体質をしており、現代的な高カロリー食が増えたことで痛風が急増しました。
- 日本人は尿酸を出しにくい?: 欧米人と比べると、日本人を含めた東アジア人は「尿酸を体外に排出する力が弱い」タイプの遺伝子を持っている人が多いとされています。
- そのため、そこまで太っていなくても、少し生活習慣が乱れただけで尿酸値が上がってしまいやすいのです。
カギを握る原因遺伝子「ABCG2」
私たちの体の中には、尿酸を外に追い出す「ポンプ」のような役割を持つ遺伝子(ABCG2など)があります。日本人は、このポンプの力が生まれつき弱い変異を持っている人が多く、特に20代などの若さで痛風を発作を起こす方は、この遺伝子の影響が強いことが分かっています。「親や親戚に痛風の人がいる」という場合は、この体質を受け継いでいる可能性が高いため、より注意が必要です。
- 治療方針
当院では、国のガイドラインに基づき、お仕事で忙しいビジネスパーソンのライフスタイルに合わせた治療を行っています。
痛風の治療は「いまある激痛を抑える治療」と「根本原因の尿酸値を下げる治療」の2ステップで進めます。
① まずは激痛を抑える(急性期の治療)
足が真っ赤に腫れて激痛が走っているときは、尿酸を下げる薬を急に飲んではいけません。 体内の尿酸値が急激に上下すると、関節にある尿酸の結晶がさらに剥がれ落ちて、痛みが悪化したり長引いたりしてしまうからです。
- 前兆があるとき: 「あ、明日あたり痛くなりそうだな……」というムズムズする前兆の段階では、コルヒチンという薬を飲むと発作を未然に抑えることができます。
- すでに激痛のとき: 痛みのピーク時は、痛み止め(NSAIDs)を短期間だけ多めに飲んで一気に炎症を抑えます。胃腸や腎臓が弱い方には、ステロイドの薬を使うこともあります。
② 痛みが引いてから根本治療(慢性期の治療)
発作が完全に治まってから(目安として1週間以上経ってから)、尿酸値を下げる薬をスタートします。
- 目指す目標値: 尿酸値を6.0 mg/dL以下に保つことを目指します。
この数値を維持し続けると、関節に溜まってしまった尿酸の結晶が数ヶ月から数年かけて少しずつ血液に溶け出していき、最終的には発作が起こりづらい体になります。 - 薬の選び方: 痛風発作は尿酸が体内で作られすぎているタイプ、尿から出にくくなっているタイプがあります。
最近はその両方に働きかける尿酸降下薬(フェブリクなど)が選ばれることが多いようです。 - 発作が再発しないよう、ごく少量から始めて何ヶ月もかけてゆっくり下げていくのがポイントです。
③ 無理のない生活習慣のアドバイス
薬と同じくらい大切なのが日常生活のケアです。当院では、忙しいビジネスパーソンでも実践しやすい工夫をご提案しています。
- 水分を1日2L以上飲む: 尿の量を増やして、尿酸をどんどん外に出しましょう(甘いジュースは逆効果です)。
特に夏場は脱水になりやすく、水分摂取が重要になります。
- お酒は「種類」より「量」: 「ビールはプリン体が多いからハイボールなら大丈夫」と思われがちですが、実はアルコールそのものが体内で尿酸を作らせ、排泄を邪魔してしまいます。
種類に関わらず、お酒全体の量を減らすことが大切です。
- 最後に:痛風は体からのSOSサインです
痛風の本当に怖いところは、「足の痛み」だけではありません。
高い尿酸値をそのまま放っておくと、尿酸の結晶が腎臓に溜まって尿毒症を引き起こしたり、高血圧や糖尿病などを合併して心筋梗塞や脳卒中のリスクを高めたりすることが分かっています。
痛風の発作は、体が「生活習慣を見直して!」と発している重要なサインです。

How is everyone holding up in this sudden surge of heat? Today, I’d like to talk about gout attacks, which are particularly common during this hot season (which, coincidentally, is also the best time of year for a cold beer!).
Gout (hyperuricemia) was once labeled a “rich man’s disease,” but in modern Japan, it has become a very common condition that can suddenly affect anyone.
Even here in our business district, we frequently see working-age patients rushing in, saying, “I woke up this morning with excruciating pain in my big toe and can barely walk!”
In this article, I will explain who is prone to gout, discuss the latest genetic insights, and outline our clinic’s specific approach to treatment in an easy-to-understand way.
- What Age Group is Most Affected? Recent Trends
Currently, over 1 million people in Japan are receiving treatment for gout. Furthermore, an estimated 10 to 20% of adult men (about 10 million people) have “hyperuricemia”—a state where blood uric acid levels are higher than normal—making them potential candidates for the condition.
Peak Onset: Men in Their 30s to 50s
Gout attacks occur most frequently in working-age men between their 30s and 50s.
- Onset at Younger Ages: While gout was traditionally associated with individuals in their 50s and older, we have recently seen a noticeable increase in cases among people in their 20s and 30s. This is thought to be driven by the Westernization of diets (such as overconsuming meat) from a young age and a rising prevalence of obesity.

The female hormone estrogen helps promote the excretion of uric acid through urine. Because of this, premenopausal women maintain lower uric acid levels and rarely develop gout.
However, after menopause, estrogen levels drop sharply, causing women’s uric acid levels to rise more easily. In fact, most female gout patients are in their 60s or older. That said, even young women should exercise caution, as excessive alcohol consumption, high intake of sugary drinks or sweets, and dehydration from extreme diets can lead to elevated uric acid levels.
- It’s Not Just Overeating! The Secrets of Genetics and Ethnicity
Gout is often misunderstood as a self-inflicted disease caused by overindulging in rich foods and drinks, but that is a misconception. Recent research has revealed that individual constitution (genetics) plays a strong role.
Ethnic Differences
The ease with which uric acid levels drop varies according to inherent ethnic characteristics.
- Oceanian Populations: The highest prevalence of gout in the world is reported among groups such as the Maori of New Zealand. They are genetically predisposed to retaining uric acid in their bodies, and the influx of modern, high-calorie diets has caused a sharp rise in gout cases.
- Are Japanese People Less Able to Excrete Uric Acid?: Compared to Westerners, East Asians—including Japanese people—are more likely to carry gene variants that result in a lower capacity to excrete uric acid. Consequently, even without being significantly overweight, slight disruptions in lifestyle can easily lead to raised uric acid levels.
The Key Gene: “ABCG2”
Our bodies have genes (such as ABCG2) that act like “pumps” to flush out uric acid. A large proportion of the Japanese population naturally carries a variant that weakens this pump function. This genetic influence is particularly strong in individuals who experience gout attacks at a young age, such as in their 20s. If you have a family history of gout, you are more likely to have inherited this trait, so extra caution is recommended.

- Choosing the Right Medication: Gout attacks can stem from overproduction of uric acid or reduced excretion. Recently, uric acid-lowering drugs that address both mechanisms (such as Febuxostat) are frequently chosen. The key is to start with a very low dose and gradually reduce levels over several months to avoid triggering a relapse.
③ Practical Lifestyle Advice
Lifestyle care is just as important as medication. We offer practical tips that even busy professionals can easily incorporate into their routines.
- Drink at Least 2 Liters of Water Daily: Increase your urine volume to help flush out uric acid (sugary juices have the opposite effect). Staying hydrated is especially critical during the summer, as dehydration occurs more easily.
- Focus on “Amount” Rather Than “Type” of Alcohol: People often think, “Beer is high in purines, so switching to a highball is fine,” but alcohol itself promotes uric acid production and hinders its excretion. Regardless of the type, reducing your overall alcohol intake is what truly matters.

The truly alarming aspect of gout isn’t just the pain in your foot. If high uric acid levels are left untreated, uric acid crystals can build up in the kidneys, potentially leading to uremia. It is also known to accompany conditions like high blood pressure and diabetes, significantly increasing the risk of heart attacks and strokes.
